Must be received in the 4-H Office by July 1. A separate application is required for each achievement pin.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Requirements to meet before applying:<\/h2>\n\n\n\n<ol>\n<li>An Achievement Pin may be awarded one time only in each Program area.<\/li>\n\n\n\n<li><a href=\"https:\/\/extension.wsu.edu\/snohomish\/4h\/members\/awards-scholarships\/achievement-pins\/\">Achievement Pin Guidelines<\/a><\/li>\n\n\n\n<li>Applying member:\n<ol>\n<li>Must be in the 5th grade or above<\/li>\n\n\n\n<li>Must be currently enrolled and be able to show evidence of active participation in the Snohomish County 4-H project for a minimum of two years (some projects require longer participation).<\/li>\n\n\n\n<li>Must have completed at least 2 years of active 4-H club work within the county (current year not included)<\/li>\n\n\n\n<li>Cloverbud member years (grades K-2) does not count toward the 2 year minimum<\/li>\n\n\n\n<li>Juniors meeting the two-year requirement may apply for one Achievement Pin<\/li>\n\n\n\n<li>Intermediates may apply for up to three Achievement Pins each year.<\/li>\n\n\n\n<li>Seniors may apply for an unlimited number of Achievement Pins if the leader feels they are eligible.<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n\n<\/div>\r\n\n<\/div>\n\n<div class=\"wsu-row wsu-row--single\" >\r\n    \n<div class=\"wsu-column\"  style=\"\">\r\n\t\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Part One<\/h2>\n\n\n\n<p>Part One to be completed by 4-H member (applicant).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">4-H Member Steps to Complete Achievement Pin Application:<\/h3>\n\n\n\n<ol>\n<li>Prepare a document with the bulleted information below (e.g. Word document, handwritten page, etc.):\n<ol>\n<li>Project<\/li>\n\n\n\n<li>Years enrolled in project<\/li>\n\n\n\n<li>Your Name<\/li>\n\n\n\n<li>Your Birthdate _____\/_____\/______<\/li>\n\n\n\n<li>Grade as of October 1 of current 4-H Year<\/li>\n\n\n\n<li>Your 4-H Club _______________________________________<\/li>\n\n\n\n<li>Best Method of Contact for you: Email | Phone | Text<\/li>\n\n\n\n<li>Your 4-H Leader | Project Leader<\/li>\n\n\n\n<li>Best Method of Contact for 4-H Leader: Email | Phone | Text<\/li>\n<\/ol>\n<\/li>\n\n\n\n<li>Write \/ type an explanation of why you feel you should receive this award.<\/li>\n\n\n\n<li>Sign your application and date it, to indicate that you have read and&nbsp; understand the guidelines for the achievement pin for your project.<\/li>\n\n\n\n<li>Choose one of the following options. (choose one only)\n<ol>\n<li>Record Book: Submit an up-to-date 4-H Record Book including Calendar, Project Record, Permanent Record and 4-H Story. Project Record includes: Commitment, Journal, Project Add Sheet (if required), and Highlights.<\/li>\n\n\n\n<li>(Highlights and 4-H Story \u2013 may be in rough draft form)<\/li>\n\n\n\n<li>Interview: You may choose a personal interview. You will be notified of your interview appointment time.<\/li>\n<\/ol>\n<\/li>\n\n\n\n<li>Give the document, including your answer to question #2, and your record book (if applicable) to your 4-H Club Leader \/ Project Leader. They will complete Part Two and submit the entire application by July 1 to the 4-H Office.&nbsp; Be sure to give your 4-H leader time to write your recommendation before July 1st.<\/li>\n<\/ol>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Part Two<\/h2>\n\n\n\n<p>Part Two to be completed by 4-H Club Leader or Project Leader<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">4-H Leader Steps to Complete Achievement Pin Application:<\/h3>\n\n\n\n<ol>\n<li>Receive Part One of the application from 4-H member, including Record Book, if applicable.<\/li>\n\n\n\n<li>On a separate sheet, explain why this member should be considered for this Achievement Pin. Emphasize the 4-H member\u2019s accomplishments and activities. Include all 4-H years from first through current. Listed below are all the areas in which the member will be evaluated. Please address these as thoroughly as possible.<\/li>\n<\/ol>\n\n\n\n<h4 class=\"wp-block-heading\">Main Project<\/h4>\n\n\n\n<ul>\n<li>Describe growth and progress within the project and involvement in project activities.<\/li>\n\n\n\n<li>In what learning experiences has this member participated?<\/li>\n\n\n\n<li>Explain the depth and breadth of project knowledge and increasing skills proficiency<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">Experiences in Leadership<\/h4>\n\n\n\n<p>Explain how responsibilities were fulfilled in the following areas.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Volunteer Leadership \u2013<\/h5>\n\n\n\n<p>Include club, county and non 4-H leadership in school and other organizations (e.g., church)<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Elected\/Appointed Leadership \u2013<\/h5>\n\n\n\n<p>For example: Club offices, county offices, committee chairs, committee participation.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">4-H Promotional Leadership \u2013<\/h5>\n\n\n\n<p>How has this member promoted the 4-H youth program?<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Experiences in Citizenship<\/h4>\n\n\n\n<ul>\n<li>Explain citizenship\/community service activities in which this member has participated. Explain the responsibilities and successes.<\/li>\n\n\n\n<li>Non 4-H &#8211; Service activities involved in as a result of church, school or other non 4-H participation<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\"><strong>Additional Information<\/strong><\/h4>\n\n\n\n<ul>\n<li>Are there any other circumstances that should be considered when evaluating this member for this award?<br>Please address disabilities, hardships, etc.<\/li>\n\n\n\n<li>Has this member attended meetings\/activities regularly?&nbsp; If not, please state the reason.<\/li>\n\n\n\n<li>If a Record Book was submitted, please verify that Record Book is complete and accurate and that the member has recorded in their Permanent Record all of their 4-H experiences (list of projects in Project Summary leadership skills, community service, club and 4-H experiences) for the current and prior years. Refer to the individual program Achievement Pin Guidelines for specific requirements.<\/li>\n\n\n\n<li>Submit Part One, your responses to Part Two questions, and Member\u2019s Record Book, if applicable, as one packet.<\/li>\n<\/ul>\n\n\n\n<p>Be sure to include:<\/p>\n\n\n\n<ul>\n<li>The name of the 4-H member you are recommending<\/li>\n\n\n\n<li>Your name and Signature<\/li>\n\n\n\n<li>Preferred method of contact for you.<\/li>\n<\/ul>\n\n\n\n<p>Mail or deliver to: 4-H Office | WSU Extension, 6705 Puget Park Drive, Snohomish, WA 98296.<br>The 4-H Office is open weekdays, 8:30 am to 4:30 pm.
