Certificate of Appreciation Recommendation Form

Use this form to recommend that an individual or organization be recognized with a Certificate of Appreciation from Yakima County 4-H. Certificates of Appreciation are awarded to individuals and organizations that have supported Yakima County 4-H in 2025. Nominees will be recognized at the 4-H Awards Dinner on Sunday, November 2nd 2025. Deadline to submit nominations is Monday, October 13th 2025.
