General Record Book Forms

Instructions on how to use these downloadable and fillable record documents:

Click on the link to the record book(s) you want to use. You will be redirected to the WSU Publications page.  Click the “Download Now” button. It will save the record book in your downloads. Open up the Record Book and you can begin typing directly into the book. Remember to save the record book on your computer or it won’t save any changes you have typed in the form. You can open the file from your computer and continue adding information to the Record Book.

Record Books

There have been some changes to the 4-H Record Book expectations and scoring.
The County Extension office is in the process of revising the book to make the record keeping simpler.
Please read the following score sheet information very closely for the details on what is expected in the 4-H record books as things are changing.

• C0934 – Level 2 Record Book and replace page 14 with the proper Project Add Sheet Record
• C0935 – 4-H Permanent Record Book, Level 2
• C0947 – 4-H Project Record, Level 2
• C0932 – Level 1 Record Book (Use for Cloverbuds)
• C0957 – 4-H Project Record, Level 1
• C0933 – 4-H  Permanent Record Book, Level 1

• 8 – Quarterly Health Feeding Record (PDF) – Optional Form
• 9 – Project Highlights (DOC)
• 10 – Project Photos (PDF)
• 11 – Yearly Inventory (DOC) – Optional Form
• 12 – Project Financial Summary (DOC)
• 13 – Level 2 Permanent Record Book (DOC)
• 14 – My 4H Story (DOC)
• 15 – Supplemental Agreement (DOC) – Optional Form
• 16 – 4-H Year Agreement (PDF) – Optional Form
