Purpose
The purpose of this annual award is to recognize a 4-H member that has shown outstanding 4-H club involvement, leadership, community service, and participation in 4-H activities beyond club work. An Extra Mile 4-H member knows the 4-H pledge and is an example of a youth member that follows its meaning in 4-H and in his or her daily life.

- The nominator submits this completed questionnaire.
- The awards committee will contact the applicant to schedule either an in-person or telephone interview if necessary to break a tie.
