This one-hour training will outline the foundations of the primary care behavioral health (PCBH) model of integration and discuss a variety of clinical applications to why meeting patients where they are at and working within the primary care system provides holistic and quality to patient care. The presenter will also provide statistics and case examples highlighting how this approach covers gaps in our healthcare system and works towards true population health strategies.
