4-H Teen Leader of the Year Nomination

Purpose

The purpose of this annual award is to recognize and encourage teen 4-H leaders who take special interest in and offer special help to 4-H members, clubs, at county 4-H activities, and in the community affairs.

Details

  • Teens must be in the 8th grade or above.
  • Teen is nominated by an adult, 4-H volunteer, or the 4-H Office staff by submitting this form.
  • The Awards Committee will contact the nominee to schedule an interview that will be held before the County Awards event.
  • Each nominee is required to submit a one-page resume at the time of the interview. Record books are not a requirement for this award.
  • A 4-H teen member can only win this award once.
  • Nomination Form must be submitted by the first Friday of October.
