Thurston County 4-H Family of the Year Nomination Form

Purpose

The purpose of this annual award is to recognize a 4-H family that has gone the extra mile for the Thurston County 4-H Program. Going the extra-mile could include volunteering a lot of time to 4-H, successful fundraising, offering unique educational opportunities to youth, participating in county and project level decision making, etc.

Details

  • One member of the family must be a currently enrolled 4-H member or adult 4-H Volunteer in Thurston County.
  • To be eligible for the award the family must be nominated by completing and submitting this form.
  • The award is to recognize a family (which is defined as any combination of immediate and/or extended family members – parents and children, grandparents, siblings, etc.)
  • A 4-H family can only win this award once.
  • The Nomination Form is due to the 4-H Office by the first Friday of October.
